Our organization is looking for a Case Navigator who can objectively analyze a situation and make determinations that are best for our client population.

Services include outreach, assessment, intervention, and referral and placement services for homeless and street-oriented community members in the Greater Tacoma communities; and other related job duties as required. This position is part of a Community network of organizations supporting our underserved population in achieving more stable living situations and to connect with valuable resources.

HOURS: 40 Hours/week

REPORT TO: Community Outreach Supervisor

Duties and Responsibilities:

· Schedule monthly or as needed interactions with clients, partners, resources and families involved in ongoing cases.

· Work with clients to help them navigate and to give clear understanding of how to create a more productive environment for themselves, as well as family members, especially their children

· Provide counseling to clients and family members, and schedule follow-up meetings with families that have been reestablished

· Collaborate with other public resources to get the best results possible for each client and family

· Attend and schedule case summary reviews and other official procedures involved in the resolution of each open case file

Job Skills and Requirements:

  • Communication Skills: Case navigator will need to be able to clearly communicate with our clients and must understand their needs. You will also need to speak with others on our client's behalf, and act as an advocate for them. Good listening skills, and speaking in an understandable way and being a good communicator is crucial to our underserved population.

  • Compassion: Case navigator will be working with people who are sometimes in dire situations or going through difficult transitions. Being compassionate to their situation and working with them in a positive way is an absolutely vital component in helping our client population make good forward strides and restoring their lives.

  • Time-Management and Organizational Skills: Case navigator will have multiple clients with various needs and are in various stages of need. There is a lot of paperwork that will need to be filled out and required reports; you will be also working with multiple agencies, as well as other internal partners. You will need to be organized and efficient with your time.

  • Problem-Solving Skills: Clients are looking for someone to help them navigate, and find solutions to their problems. You will need to think critically and clearly about their situations to assist them in navigating and finding solutions.

Qualifications:

· Bachelor's Degree in Social Work required

· Valid state counseling license required

· Proven time management skills

· Must have a valid driver's license with clean driving record

· Basic understanding of Microsoft Word and Excel required.

· Management and able to track data of clients

· Homeless Case Management Tool – HMIS Data Entry Software. System works with other state agencies providing services for the homeless community.

· Able to use smart devices with MS tools (i.e. smart phones, tablets, computers...)
