We offer a variety Community Outreach initiatives, aimed at serving those struggling with housing and food instability across Pierce County. We are looking to fill our Community Outreach Coordinator role with someone who has a passion for serving the community.

The Community Outreach Supervisor will assist the Executive Director to build relationships with homeless neighbors, provide advocacy, and assist with connection to benefits and housing.

The primary goal is to identify unsheltered homeless neighbors through direct street outreach activities and through community referrals from mental health agencies, hospitals, local law enforcement and municipalities and other social serve agencies. The outreach team will administer assessment tools, facilitate placement into emergency housing if needed and connect individuals to social services and permanent housing. The outreach team will also assist neighbors in obtaining housing readiness documentation and accompany them through the housing application process.

HOURS: 40 Hours/week

Duties and Responsibilities:

· Work collaboratively with the Homeless Outreach Manager to develop and implement the R.I.S.E Homeless Outreach Program

· Maintain a calendar of outreach activities, including community events, workshops, event appearances, and other communications opportunities

· Conduct routine/periodic visits (Pop-Ups) to homeless shelters and encampments to share information and support face to face

· Work with Navigators from other agencies in the area to provide team outreach information services. Collaborate with new and present partners

· Attend team meetings, case conferences, training workshops and community meetings as needed

· Maintain complete client records, daily activity logs, mileage logs another report as directed

· Maintain HMIS management system and data input

· Organize special events and fundraising plan

· Establish and maintain positive, productive working relationships with mental health programs, shelter programs, police (and other officials), and providers of services and resources to homeless neighbors

· Participate and lead homeless task force

· Assist in obtaining housing readiness documentation such as ID, social security card and income verification and work with Transportation Navigator to provide transportation to appointments as necessary

· Assist in identifying appropriate housing and completing housing applications

· Assist homeless neighbors with accessing resources and making referrals; continue to engage with unsheltered homeless neighbors until shelter or permanent housing is obtained

· Provide support for people as they transition from homelessness to housing

· Provide advocacy for homeless neighbors when they encounter barriers

· Provides internship overseeing and coordination of internship and staff working to achieve common goals

Qualifications:

· BA in social work or related field or 2 years of equivalent social services experience

· Exhibits dedication to the mission and vision of restoring community

· Basic understanding of severe mental health, physical health and substance abuse issues and symptoms

· Listen with sensitivity to individual’s feelings, needs and point of view; demonstrates tact and courtesy in expressing options or ideas, and recognizing opportunities to enhance community relations

· Proficient with computers and related technology; must maintain neighbor related data tracking systems including case notes

· Maintains a non-judgmental attitude and a display of unconditional positive regard, is caring and compassionate

· Displays appropriate, professional, healthy boundaries and exercises mature judgment and understanding of safety concerns

· Completes tasks in a timely and efficient manner, utilizing good organizational skills. Has the ability to manage multiple cases efficiently and effectively

· Adapts well to changing priorities and challenging individuals

· Effectively works within a team and successfully motivates others

· Maintains personal and professional integrity, and handles confidential information with discretion

· Desires to grow in knowledge of homeless services and resources

· Possesses a valid Washington driver's license, proof of insurance and a clean driving record

· Management of data/material and able to track data of clients/contacts

· Homeless Case Management Tool – HMIS Data Entry Software. System works with other state agencies providing services for the homeless community.

· Able to use smart devices with Microsoft Office tools (i.e., smart phones, tablets, computers)

Accountability and Review:

3-month probationary period with a performance review, followed by annual evaluations

Employees will be subjected to outside environmental conditions while conducting various levels of outreach to homeless neighbors. In addition to working in a typical office environment, the navigator will be required to work in areas inhabited by homeless people, to include working in dirty environments and dealing with challenging individuals
